My students submit this form at the end of a unit. It's a quick summary of their quiz and review scores. I use the notes column to confirm they've taken adequate and appropriate notes for each lesson. You might use the column to check off when they've reviewed their notes instead. The space at the bottom is for comments or questions from the student or instructor. I like to write encouraging statements to my students there too!
