This is my 12th year teaching 4th grade in the public school system. I student taught in a 5th grade classroom and have worked with students of all needs from special education to gifted and talented.
My students must complete this form when their card is moved to orange on my behavior chart; however, this form can be modified to fit most behavior plans. It is important for students to reflect on the rule(s) that they broke, their consequence(s), and how they will improve in the future. They must take this form home and have it signed by a parent.
The educational process works best when all members can communicate! You can have great parent-teacher communication with this document. Students can complete this form to reflect upon the week. Teachers can evaluate their students' behavior and effort and notes can be written from the student, teacher, and parent for home/school communication.
This half-sheet resource (formatted to fit a reading notebook) is perfect for helping students jot their thinking about their independent reading. It gives them sentence starters to record their thinking about skills such as predicting, visualizing, and forming ideas about characters.
