This form can be printed double sided and cut in half to save paper. I graph the data based on students weekly math and ELA assessments and send it home the following Monday. This way parents see their child's progress and see their score compared to the whole class. I don't have to worry about sending the test home and not getting them back. Print one master fill in the graph, make copies, cut and send it home. I hope this helps.
