My district recently began using the 5D+ Rubric for Instructional Growth and Teacher Evaluation ™. Under the assessment dimension, students should have frequent opportunities to assess their own learning in relation to the success criteria and be able to use the success criteria for improvement. I created this worksheet to improve my practice in this area. This resource can be used as an exit slip across subjects. It would also be valuable to use it with the same learning target across time
