The chart comes with 17 behavior targets that can be used to teach appropriate behavior. I have included two versions of the chart. One just has stars and the other has stars and descriptions of what the number of stars means. This chart is designed to teach students to monitor and change their behavior in a positive way and isn’t used as a disciplinary chart. It has helped increase positive behavior in my classroom. There is a different reading log for each month of the year. I have included three different formats of the logs. One set has a place for the date, title, minutes read, and parent initials. Another set of logs has a place for the date, title, and check off box if they took an A.R. quiz (no parent initial column). The third set has only date, title, and minutes read. They each have 24 spaces to write titles.
