Star of the Week form!I choose a Star of the Week each week for 12 weeks (I have 12 students). Each student is sent home with this form 1 week prior to their assigned week. This is a great way to keep behaviors under control and motivate others! Students bring in a poster decorated however they like and it is displayed all week. This is the form I use to notify parents and give directions of what to do! Enjoy!
