I used this Reading Log sheet as nightly homework with my second graders. They are allowed to pick any book they wish. All they need to do is write the title of the book they read, and all parents have to do is sign. I am not big on homework, but this let me know that they were reading each night, which is very important! There is a spot to fill in how many minutes they need to read each night.
