I AM REALLY HAPPY ABOUT HOW THIS TURNED OUT.
This is a gamification of a PowerPoint Quiz. You can see a video preview of how it functions here:

Students get 2 strikes and the third takes them out but they can start again. Slides show how many strikes they have and with each strike brings them to a different set of questions.
