This weekly Basketball Behavior Plan includes space for you to write in specific ways that you want to see improvement in the child's actions, explains the game plan, space for you to write in what the rewards and consequences are, and the goal. Behavior is tracked and recorded Monday through Friday with a place for making behavioral notes, praises, or areas of concern. Each day, there is a place for a parent signature so parents stay in the loop for how behavior was each day.
