This behavior chart was created for a student who has trouble staying on task in class. I came up with three goals (which can be changed for each students’ needs) with the student and a goal for each day on the amount of goals he needs to reach. After each part of the day, I discuss with the student how well he did at succeeding at each goal and give him a sticker for each goal that he earned. The chart is sent home each night for his father to sign.
I created this for my evaluation by my Assistant Principal. She suggested using a chart to record observations to help assess students on how they did during the lesson.
In the first column, I will be putting my student’s names. The next three columns are for checks for students who successfully participated in and understood activities or concepts. There are three columns to assess a total of three activities in one lesson. I have also included a column for any notes the teacher wishes to
