I use this product in my classroom at the beginning of the year to match my readers with books that align with their interests. This product helps to give insight into their interests and previous reading habits.

This sheet can be used to notify parents of their child's behavior throughout the week. It includes a spot for goals for the coming week, as well as any positive or negative points for each day.
