This is an application for students wanting to run for student council. It asks for information such as other extracurricular responsibilities, grades in current classes and their intentions for becoming involved in student government.
5th - 12th
Late Work Pass
This item can be edited to fit your classroom or school logo. I give students 2 passes at the beginning of each marking period to use when their extra-curricular activities impede their ability to get work done.

Oops Slip
This item is for late elementary, middle school or high school students who do not complete their homework. They can reflect on their reason for missing work and make a plan for how they can improve. Teachers can keep the "oops slips" for conferences or parent meetings. It's an easy way for students to be accountable. It also is a non-confrontational way to show parents why their student is not completing assignments as well.

I have worked in many positions at the middle school level, including special education and regular education language arts and math. Presently, I am teaching 6th grade math and reading.
Teaching style
I believe school should be relevant and meaningful to students as well as fun and engaging. I strive to teach with various interesting activities, most of which are reflective of how the concept or skill may be used outside of the classroom. In addition, I always consider humor and laughter as the best mode of delivery.

I am certified 6-12 in math and English. I also have a master's degree in Special Education with certifications in cognitive impairments and learning disabilities.
