This is a form for parents to sign up for student-led conferences. Use the form for student-led or regular conference styles by editing. The time slots are designed to accommodate 2 families at once, allowing each family 10 minutes at the beginning or end of the conference for confidentiality. Parents rank their top 3 choices for potential time slots.

Organize your students into flexible groups for writing! There are spaces for 6 groups based on Six Traits of Writing- ideas, voice, organization, sentence fluency, word choice, and conventions. Group students by which trait they need to work on most, or just use the trait group names as a way for students to remember all 6 traits.
