The Behavior Chick

This is an executive functioning assessment that I use for my high school students. I have found most students need help in several areas of executive functioning. This assessment includes these areas: Impulse Control, Emotional Control, Planning/Prioritizing, Flexibility, Working Memory, Self-Monitoring, Task Initiation, Organization, Attentiveness, Time Management and Social Awareness. It's helpful for figuring out which areas to target when writing IEP goals.

This is a Google Slides activity in which students practice basic map skills. Topics include: using the map key/legend, compass rose, labeling maps, landforms, bodies of water and differences/similarities in maps and globes.

This is a 2 page assessment to see what angers/frustrates your students. This includes 110 different items to rate and it's editable.

This is an informal transition assessment I use to get my students thinking about their future. The information from this assessment can be used when writing IEPs or if you just want to get to know your students more!

This is a behavioral observation form to use with students. It's editable and mostly requires just checking boxes with minimal writing, if wanted. I use it to track and observe my students with emotional/behavioral needs to help collect data. The data can be used in writing IEPs/FBAs and BIPs.

This is a fun Google Form to have students complete. It has pictures and words and be used for multiple grade levels. It includes a mixture of interest questions (i.e. favorite color, favorite food, etc.), as well as open-ended questions (i.e. if you could live anywhere in the world, where would it be and why?). This is useful information for learning about your students, finding out their interests/preferences, as well as what type of reinforcers might work well for each student.

This is a quick and easy checklist-style form to fill out when a behavioral incident occurs. It is editable and only takes a minute to complete. I use this form to collect data on individual students for IEPs, FBAs and BIPs, as well as data in general.

This sorting activity can be used to reinforce and practice differentiating between primary and secondary sources. You can make a copy for each student in your class to submit and it's easily editable. I used this with high school students with Multiple Disabilities to review.

I use these sheets for progress monitoring for IEP goals. It makes data collection easy and can easily be printed and put on a clipboard or in a binder.

This is a fun Bingo game I use with my high school Emotionally Disturbed students. This Bingo game helps students remember/learn various ways to cool down when they are angry/upset. There are 25 different Bingo boards included.
