This is an executive functioning assessment that I use for my high school students. I have found most students need help in several areas of executive functioning. This assessment includes these areas: Impulse Control, Emotional Control, Planning/Prioritizing, Flexibility, Working Memory, Self-Monitoring, Task Initiation, Organization, Attentiveness, Time Management and Social Awareness. It's helpful for figuring out which areas to target when writing IEP goals.

This is a fun Google Form to have students complete. It has pictures and words and be used for multiple grade levels. It includes a mixture of interest questions (i.e. favorite color, favorite food, etc.), as well as open-ended questions (i.e. if you could live anywhere in the world, where would it be and why?). This is useful information for learning about your students, finding out their interests/preferences, as well as what type of reinforcers might work well for each student.
