I love this test because a calculator would be completely useless!
3 versions of a 3 page test from Pre-Calculus (prevent cheating, give retakes)
Students are asked to find the EXACT value of sin/cos/tan of angles using different formulas.
Simplify and factor trigonometric trinomials
Solve trig equations for exact general solutions.
Create their own trig identity (but there's a catch... it has to simplify to a word)

Students are graded on Timeliness, Completion, Accuracy, & Labeling. Not all categories have equal weights. This saved me when I was struggling to grade homework on time it really lifted a weight off of my shoulders.

Sequences & Series Test (4 versions)

Entire test is Short Answer / Free Response
4 versions (prevents cheating and different versions can be used as retakes)
Each test is 2 pages and of equal difficulty.
Sequences on 1st page, Series on 2nd page
There is some overlap in questions however the majority are unique to that particular version
Points add up to 20 making it easy to calculate students percentage grade without a calculator.

This is probably the best thing I've created, my students love it and it really helps organize all of the information!!
I have a personal preference for the chart however a few students liked the web more (its all the same information).
*I refer to -sin(x) as a VERTICAL reflection across the x-axis. I know that currently this is considered a horizontal reflection but I'm working on a paper to make an argument for why this shouldn't be. In the art

"I Like Fun" a book for Kindergartners
I put together this book uses the words that my daughter had been learning in from preschool through the first 3 months of kindergarten. A lot of family liked it so I decided to put it here. I'm not an artist so it contains public images that were pulled from the internet. It's not a polished product but it is definitely a fun read that children should be able to read completely independently once they learn the sounds/words. The format is a slideshow. This is an 11 question Scavenger Hunt. Each answer is a room number in the building where students would go and then find the next clue hidden (barely) somewhere within 6ft of the door in an envelope that I labeled S^2. All of the problems are related to Systems of Equations & Matrices and students love it because they get to explore the building often to places they haven't been yet.

Each problem is connected to at least one of the previous problems and ultimately tell the story of a love affair between the characters of "Romeo & Juliet" vs "Rapunzel".
Most of the answers are "pretty" (clean whole numbers, this is a norm in my classroom and I make an effort to do this so that students feel more confident about their final answer and if its not a whole number then maybe they did something wrong)
