Here is a test for Northeast Region of the United States. Students will need to label states and capitals. I post a blank map up on the overhead projector that is numbered 1-11. Students need to match the state name with the correct state on the map. Then they need to match the correct capital with the state.

All you need is your own map of the Midwest that you label 1-12. Students need to match up the state with the correct state represented on the map and then match the capital with that state! I have used these all year for all the different regions and it has been working out great. If a student needs to retake the test you can switch around your numbering of the states! Easy peasy lemon squeezy!
