I created this form to use with my 2nd grade students. Previously students had to "walk laps" as punishment at recess. This proved to be an ineffective consequence for many. Now my cohorts and I rotate hosting Recess Detention and Recess. Students fill out this to document behaviors. These forms can be filed in each student's personal file or kept in a notebook depending on how you personally want to manage it.
