Have students fill out one of these when they are missing a HW assignment. It gives them an opportunity to explain an extenuating circumstance that may excuse the HW and it gives them an opportunity to reflect and be proactive in the prevention of further missing assignments by identifying the problem and creating a plan to solve it. Once the missing HW is turned in, I staple the student reflect to the HW so parents will know how it was addressed with the student.
