I found that as I was completing the activities in Eureka math my students would benefit from having the Concept Development problems written out. This allows me to differentiate learning and allow students who are ready to work in collaborative groups or independently to work on the problems while I work with students who need more guidance. This also allows students to have the problems in front of them and not projected or written on the board.

I use this assessment and goal setting sheet prior to starting instruction on the 3rd module in Eureka math. It provides my students and me with the information I need to begin planning for instruction related to these standards. After the students take the assessment and I grade it they fill in the goal-setting form to set their learning targets for the module based on their individual results.
I also used this assessment at the end as a "study guide" for the final module text so the student
I use this as a pre-assessment prior to beginning the module. The CCSS learning targets as well as success criteria are clearly listed for the students to allow for visible learning. I then follow this up with the same assessment as a "study guide" to collect pre and post assessment data.
