This is a sheet of paper where you can record common, but disruptive, behavior issues per student. There are places to date and record information of the students actions, as well as your own. There is also room for comments to keep an accurate record of your own. It is great for meetings with administration, teams, and/or parents.
This is a blank template that works with reading skills including:
main and supporting characters; rising action, climax, and falling action events and descriptions; theme and genre identification; summarizing; creating imagery; and identifying interesting words. You can use if for any movie and any genre.

I am not a fan of book work. Though books are an important resource, I try to incorporate some independent work, some collaborative, and some hands-on activities in my daily instruction.
