This is a sheet of paper where you can record common, but disruptive, behavior issues per student. There are places to date and record information of the students actions, as well as your own. There is also room for comments to keep an accurate record of your own. It is great for meetings with administration, teams, and/or parents.

Experience
I am a fourth year teacher at a rural WV middle school. I have taught a different aspect of the ELA Curriculum and a different grade each of my four years. This experience, though challenging at times, has been beneficial in many ways.
Teaching style
I am not a fan of book work. Though books are an important resource, I try to incorporate some independent work, some collaborative, and some hands-on activities in my daily instruction.
