This tool can be used to document how students feel about learning Math. I created this last year prior to parent/teacher conferences so I could use a student's self-reflection to begin the conversation with parents. I then placed the completed forms in student data folders. This can be used for each marking period so the students can see how their feelings towards Math change (and hopefully improve) throughout the year!
