Pink Slip - for missing homework

I don't listen to excuses, I just have them fill out the form, and I'll look at it later to see what their excuse is. I keep these on file, and they have come in handy for parent conferences (especially if a student is saying they didn't have time to do their homework due to sports or something or the sort).

It asks them to rate your teacher qualities, anonymously, on a scale of never, rarely, sometimes, usually or always. It also provides a section for students to provide their own tips. This survey will not only provide you with valuable feedback about your teaching, it will allow students to feel that their opinion is truly valued by you.
