Achievement awards that can be changed from math to any subject. I use these awards in my classroom to motivate students to increase their quiz/test scores. You can post them around the classroom or hand them out to students. I do both. The students take great pride in their name being called out in front of the whole class and students that did not earn an award are motivated to do so.

Test/Quiz Reflection Form
This is a form designed to encourage students to mindfully reflect on their test/quiz scores. I give these to my students printed on a colored paper before they recover their test/quiz grade. It allows them to reflect and then I can use it as the cover page for their test/recovery test when scores go home! Great way to encourage parents to discuss with their students about their assessments. EDITABLE so you can change to fit any type of test!

Weekly Assignment Sheet
Weekly assignment sheets are given to students to help train them to be organized. It is also an excellent communication between teacher and parent. Because students have to get them signed, parents can't claim they didn't know about the test, quiz, whatever! Use year after year, just make changes as needed. This lesson plan was made for the R.I. new evaluation system. It is extremely extensive and is chalk full of information. Along with the lesson plan there is an exit question, a differentiated quiz with criteria chart, and, directions for the students on how to make a circuit.
