Cards are punched with a hole punch for every 100 a student makes on an AR quiz. Every three punches the student is bumped up a point level in AR. Student is awarded when they reach 10, 100's. Cards can be printed on card stock and laminated for durability. These are great incentives for students to do well in Accelerated Reader. Animal logo is a frog. Teacher's name can be added in case students lose their cards.

I put any weekly quiz grades and notes about what their student needs to work on in the Academic section. I then put their behavior for the week in the Behavior section. There is a place for the parent to request a conference if they need to. This was created with Microsoft Word so you can change it if you wish.
