Lab Safety Contract

After teaching Lab Safety, I send these contracts home with the students for parent signatures. I also give a Lab Safety quiz, which the students must get 100% on in order to participate in labs. There is one copy of the contract to send home for parents, and one to keep in the students’ journals. Parents keep their top copy at home and return the signed portion. Students sign their contract, give me the signed portion and glue the top into their journals. Enjoy!
