Beginning of the Year Mathematics Survey - Includes Spanish translation

Created by
Math Inc
I have used this beginning of the year survey for my math classes over the past couple years. It's a great way to get to know students! I keep it handy for reference throughout the year! I usually copy this back-to-back with a separate document that contains math questions to serve as a formative assessment tool for our first unit. This way students can take their time filling out the survey in class and continue on to the math problems on the back if they finish before their classmates.

This fluency leads to better comprehension and enjoyment of the text. Increases vocabulary: Repeated readings help you understand the context and meaning of unfamiliar words in the text. This

Every parent loves to hear that their child did something nice. Too many times teachers get into the habit of only sending home negative notes. This is a quick note that teachers can fill out quickly and send home to parents to let them know that their child did something positive. This is a great way to increase parent/teacher communication and relationships. This work is licensed under a Creative Commons Attribution 3.0 Unported License.

After every test, I require students to fix all of their mistakes (regardless of how well they did). This form requires students to not only find a new answer, but to identify where they went wrong, so they hopefully won't make similar mistakes in the future. I allow students to get help and use any resources they can for these corrections, and they count as a quiz grade. This also helps students improve their grade a bit if they didn't do well on the original assessment. This works very well

My system is twofold – if the student will not be in to get work, I have a form that allows you to include more details. This is designed to help parents and students get information and classwork easily, even without you there to explain. Generally I attach any worksheet
