This is a quick documentation I use with the students when they are done with their math stations. They each have a folder of multiple copies of this page. After stations, I give them 3-5 minutes to reflect on what they did in their stations that day and fill out the reflection sheet. Once I week I will glance at them an put either a sticker or stamp. To let them know that I am paying attention I will read them every now and then and comment on their responses.
