This is a sheet I used with my second graders for when they did not have good behavior. Instead of having to write out what they did and what they did next time, they had a big enough area where they could draw what happened and what they would do next time. Older kids can use this as well and write in the areas instead of drawing a picture. It has a place for their parents to sign at the bottom.

There is a student with severe behavioral issues on my caseload who also requires visuals. This version of a check-in/check-out sheet really works for him because he can earn points even if he messes up a little bit, and he can see what comes next in his day. They are easy to write on if changes to the schedule happen. Suggestion: Laminate to use day to day with a dry-erase marker.

I use this template to print directly on post-it notes using my printer at home. It's a great way to send home reminders, or positive feedback to parents that will catch their eye. Each student had a copy of this in a plastic sheet protector in their center binder and would use a Vis-a-Vis marker to initial when they finished a center.
