Pink Slip - for missing homework
This is a strategy I got from Harry Wong (great presenter and author!). It's called a Pink Slip, and students fill it out whenever they do not have their homework. I don't listen to excuses, I just have them fill out the form, and I'll look at it later to see what their excuse is. I keep these on file, and they have come in handy for parent conferences (especially if a student is saying they didn't have time to do their homework due to sports or something or the sort).

Missing Homework Excuse Form for Students
Last year I created a "form" for students to fill-out when they did not have their weekly Homework ready to turn in on Friday.
This helped me when conferencing with parents. When the child had zeros on a progress report, I could easily take out the HW excuse forms filled in by the student. It did help the students feel more accountable.
You'll need to tweek it to your needs.

The Assessment for Learning Template is originally an Ontario Ministry of Education document. I recreated it for a math team several years ago and have since added columns for Knowledge/Understanding, Thinking, Communication, and Application. This is useful for taking notes during three-part lessons and other small-group math work.

Solution for struggling and underachieving students
The following is a description of 2 methods I used to help parents be more involved in their student's lives. I used it for students who were getting a C- or below but you can use it for any desirable grade. I can honestly say that I saw improvement in 68% of student's grades by the next unit test and 77% in student's grades by the end of the semester.

I am having my students record their test scores throughout the year to ensure they are learning. This allows the students to practice creating bar graphs (or a line graph) so we can see where the students scores are and where we want them to be.
