This is a little "cheat sheet" I use in small group to keep me on track of kindergarten skills that are taught during the first quarter. You can run a master copy in color and one for each student in black and white. I use the student copy to make anecdotal notes and to plan for small group instruction. It is a very useful record to have for parent/teacher conferences. I am making it editable so that you can customize the letters and sight words you teach!

I use this sheet when a student (s) cannot follow the classroom rules. This form keeps the parents up to date on behaviors being seen in the classroom and how well the student is following classroom rules. I put a sticker or star in each area so that the student knows how well he is doing. I also contact the parents to enlist their help. If the child gets all the stickers for the day, he/she goes home to a special treat from the parents.
