I use this sheet with my students on a monthly basis. They set goals for themselves, I post the form in my classroom, and they work to achieve their specific goals over the course of the month. It holds them accountable for what they need to improve upon, and allows them an opportunity to think critically.
Since implementing this form, my students have become more motivated to reflect on their learning and figure out where/how they will improve.

I used this report everyday with my students. When I introduced a new lesson I would had this paper out. Students were responsible for writing down the CCSS standards (which had to be up in my classroom as per my principal).
