At open house each fall, I would provide parents with a stack of these pre-printed "note to teacher" on a bright colored paper. It allowed parents to quickly write down notes about doctor's appointments, changes to pick up, or other notes that pertain to their child.
I found it helped parents to quickly send in notes, the school secretary loved the simple format and easy to read layout (and consistent layout), and I loved that I could easily find them in a child's bookbag.

Student Checklist
This little thing is a life-saver! Simple add your student's names (it is editable), print as many as you want (there are 2 per page.) Then write in the assignment, field trip, money due, etc. at the top of the form. Then you have an automatic checklist for whatever you need!
I use these to check students off for homework turned in, late assignments, paper clip to the top of a stack of papers and write each student's grade on the check list, etc. It is just always handy to have a check list!

Back to School Editable Pirate Themed Cover Sheet for Weekly Graded Papers
We send Take-Home Folders home once a week. There are always a stack of graded papers and several tests or quizzes that I prefer to know parents look at. In order to help out my busy parents, I ask them to sign just one page as opposed to several. I created this form for this reason. Missing Assignments Log
Hold your students accountable by having them record each of their missing assignments, explain the reason it is missing, and record how they will ensure another missing assignment will not happen again. Keep a "Missing Assignments" binder that houses 1 sheet (or more) per student. Send the binder around whenever an assignment is due, asking students to either place their assignment in the binder (temporarily) or sign the log. Review before the next day to hold students accountable.

Assignment Tracking and Grade Sheets
I created this form for tracking and recording assignments when I taught multiple high-school level classes and had student workers. I used it as follows:

Exit Slips Tickets Simple Design 3 to a Page
Excellent tool for formative feedback. Give the class a culminating question, topic or get them to ask you a question. Have them write it out and hand it to you before they leave class. 3 tickets per page to save paper. Cut them up and they are ready to go. Writing Process Checklist
