Pink Slip - for missing homework
This is a strategy I got from Harry Wong (great presenter and author!). It's called a Pink Slip, and students fill it out whenever they do not have their homework. I don't listen to excuses, I just have them fill out the form, and I'll look at it later to see what their excuse is. I keep these on file, and they have come in handy for parent conferences (especially if a student is saying they didn't have time to do their homework due to sports or something or the sort).

Use this self-evaluation to help your students conduct student-lead parent teacher conferences. Your students will be encouraged to reflect on their behavior and study habits in class. They will report their current grade and why they are getting that grade. The students will then relay that information to their parents during a conference!

Missing Homework Excuse Form for Students
Last year I created a "form" for students to fill-out when they did not have their weekly Homework ready to turn in on Friday.
This helped me when conferencing with parents. When the child had zeros on a progress report, I could easily take out the HW excuse forms filled in by the student. It did help the students feel more accountable.
You'll need to tweek it to your needs.
