For the smaller, but disruptive, behaviors in class. Have a stack of these in an accessible spot in the classroom for your students to have access to. Whenever they are doing a not-so-kind behavior, ask them to get a fix it ticket and fill it out. They need to take it home and get a parent signature, so you know a parent saw it. Have them fill out as many as you need and hopefully the behaviors stop.

I keep a stack of these charts on a clipboard- one for each group. The boxes on the side fit the 1 7/8 in square post-it notes. Under the lesson plans I have one page per student in the group with boxes which hold the sticky notes. I will add this page to my store as well. I use the sticky notes to keep anecdotal notes on students. I like to use sticky notes because I can stack them in one spot if I have multiple notes for one day.
