This is the form I keep in each Behavior Binder for students in my Primary Autism Classroom. My BII inputs daily data counts to give me a full picture for IEP and FBA writing, I can see in a glance if behavior interventions are working by the decrease in totals from week to week or month to month. I use it for 1 or 2 target Bx. Data Tallies are taken on laminated data sheets in sharpie, and erased and the end of the day using a dry erase marker (*TIP) to save paper and clutter.
