Toddler Daily Log
This colorful daily log for parents of toddlers includes spaces for diaper changes, meals, naps, and activities. It is without lines or timestamps so that caregivers may customize the schedule to their preferences. Line provided at top of log for child's name.
Suggested Uses for Toddler Daily Log:
-For home use: Print and laminate. Use dry erase marker to keep track of your toddler's day. Wipe clean with cloth at end of day.
-For daycare use: Print daily copies to send home with parents.

Individual Child Planning Form
This form is for use in a preschool or kindergarten classroom to help teachers and staff plan for child-centered and project based learning. I recommend printing a separate sheet for each student and keeping them in a binder or displayed on a bulletin board with easy access. The space in the middle is intentionally left blank for a child's name, photograph, or category left to teachers' discretion.

Speech Therapy Session Information Sheet for Parents
I use these information sheets with my students for speech. I have found that parents love these because it tells what we did in speech, what goals we worked on, discusses their child's behavior during speech, and tells whether the child is making progress.

Alternative Assessment
The Alternative Assessment document is intended to offer students choice in regards to how they want to demonstrate what they have learned. I started using this Alternative Assessment form with students that performed poorly on traditional tests. These directions offer learners the option to apply what they have learned regurgitating information for a test.
