These are small charts for students in the classroom who have a difficult time remembering math facts. They are great accommodations for students with disabilities and can be used as supplemental aids for tests and classroom work. They easily affix to the inside of notebooks or planners for easy access or can be applied directly to the surface of a desk or table.
This is a calendar for September of 2024. I use this as daily anecdotal records for different skills my students are working on. I also flip over to the back to record any behaviors or parent contacts.

This skills checklist can be used for quick documentation while students are working in centers. It can also be used as documentation to support the PK SLT Assessment. Simply write the child's name, circle the time of year (Fall, Winter, Spring), and write tally marks when you see the child perform the skill. I personally put one, two, or three tally marks to match the key on the PK SLT Assessment. This checklist has been used in my classroom successfully and can be placed in students' portfolio
Our district requires a summary sheet for each student receiving RTI services. Choose which summary works best for you, either by trimester or quarter. Print a copy for each student.
