I have used this beginning of the year survey for my math classes over the past couple years. It's a great way to get to know students! I keep it handy for reference throughout the year! I usually copy this back-to-back with a separate document that contains math questions to serve as a formative assessment tool for our first unit. This way students can take their time filling out the survey in class and continue on to the math problems on the back if they finish before their classmates.
