This is the Code of Conduct we created for Student Council. It can be used for many different types of Student Councils - whether it is younger or older students.
The only thing you will need to change is the name of your school! :)

Effort and Attitude Rubric
This allows you and your students to rate them on cooperation, teamwork, attitude, and effort. It also has a place for students to explain why they gave themselves what they did. I use it at the end of each grading cycle for my shop class but can be modified to fit many other courses.

This is a basic color coded 8 period lesson planning form for teachers to use to plan their weekly lessons in the middle or high school level classroom.
