I filled this form out whenever I subbed for a teacher. It helped me keep track and helped me tell the teacher that I subbed for exactly what we did that day in each subject. I was also able to note the children that I had problems with at the bottom. I know a lot of teachers like to know what goes on during the day they are absent! You could even change this and leave it for when you have a sub for them to leave you their information and how their day went! :)
