PBIS Check In-Check Out Sheet

Created by
Kayla Arnold
There is a student with severe behavioral issues on my caseload who also requires visuals. This version of a check-in/check-out sheet really works for him because he can earn points even if he messes up a little bit, and he can see what comes next in his day. They are easy to write on if changes to the schedule happen. Suggestion: Laminate to use day to day with a dry-erase marker.

It has ten boxes made to record a check or x on each attempt to obtain the students' goals. Just below is a box setup for any notes about the students' progress. Every special education teacher should have a system similar to this because it makes life so much easiler. Enjoy!! Mandijay10
