Speech Therapy Session Information Sheet for Parents
I use these information sheets with my students for speech. I have found that parents love these because it tells what we did in speech, what goals we worked on, discusses their child's behavior during speech, and tells whether the child is making progress.

These templates were created and intended to be used with UFLI Day 1 and Day 2 instruction. I laminate these templates front to back and students use dry erase markers.
