I know that we all recognize the value of practicing a skill we are learning, whether learning how to ski, create a soufflé, or learn to speak a new language. The

Student Observation Log

This is an EXTREMELY simple form I keep in my binders for each class. There is a copy of this form for each student. I keep a clipboard of labels nearby during class, and when I note a behavior that I want to record, or if there is an incident in class, I record it on a label. Later when I have time, I stick the label on the observation form. Now my documentation is a piece of cake. If I need to talk to parents or administrators about a student's behavior in class, I just have to look at my log.
