Digital Grade Check for High School - All Periods

I've used this online grade check form in AVID (as a Google Doc), usually at the end of a grading period, and found it helpful in identifying where students may need support. It's also a good reflection activity, as it asks them to consider their effort and habits when it comes to each class. I make sure to set it up as a support activity so they can feel comfortable in telling me how they're doing.

Behavior Tracker (Tier II Students) in Secondary School

I created this tracker for students exhibiting challenging behaviors. This tracking system was used in conjunction with positive reinforcers/incentives that appealed to the student. For example, if the student achieved a score of 100 then he or she would gain access to a preferred incentive.
