Self-Monitoring Behavior Charts
Two different levels of self-monitoring behavior charts.
How to use:
I sit down with each student and we chose 4 behaviors to monitor throughout the day. They also chose the reinforcer they will be working towards if they earn the necessary points.
After each segment, a paraprofessional or myself review if they met this expectation. At the end of day, if they earned enough points then they get their reward!

Weekly Behavior Chart
I use this weekly behavior chart with my primary students. Students color in their happy smiley faces in purple (role model) or green (great day), or the middle neutral smiley face in yellow (warning), or the frown in red (parent contact). It also has a spot for parents to sign and for teacher to comment if needed.

Focus, Refocus, Redirection-Classroom Management Strategy
Students at times may become disruptive in the classroom, however many times they are redirected repeatedly, sent to another teacher, or when you have had enough, the principal's office . Student Behavior Sheet
This is a behavior sheet I created for my students to take home every night. It correlates with my clip chart tally mark behavior sheet. Green- 0 marks, yellow -1 marks, bottom of yellow -2 marks, orange -3 marks, bottom of orange -4 marks, and red -5 marks. My Weekly Behavior log
Quick and easy behavior log for parent to teacher communication. You can have students color their smiley face or you can circle it or stamp it on your own.
