Created by

This behavior rating scale was designed for a student who has already been diagnosed with ADHD or has the same symptoms as ADHD. It isn't a screener to identify possible ADHD. I fit 2 days per page, but if you set your printer to print on both sides of the pages, you can fit 4 on one sheet of paper. Please see my other product that I personally use for my son while we proactively work with managing and and controlling his ADHD while remaining positive!

Two-Sided Behavior Chart for Daily Monitoring

Created by

Use these daily behavior charts to monitor an entire class, or specific individuals. They are two-sided: the front gives general examples of expected behaviors from anyone (editable), the backside is available specific goals to that individual (from IEP, RTI, etc.). These get passed out everyday, and collected at the end of the day (or each period.) Any teacher, para, adult-volunteer can mark the chart - I usually put a note next to any negative mark as a reminder of why they got it. If the

My Restaurant Schedule is designed to assist a high-needs, active child (e.g., moderate-severe autism or ADD/ADHD) when he or she eats a meal at a restaurant. There are step-by-step behavioral tasks that the child must complete in order to earn a “check” or a circle around a favorite image (an example sheet is included with a column of green dinosaurs). The steps are very detailed and repetitive, such as “Stay in your seat,” then “Open your menu and choose your food,” then “Stay in your seat” ag

Help Box visual for desk table work. Behavior attention autism support

Print and laminate and attach to your child's desk. Used as a visual tool mini-whiteboard. Write the "jobs" with a dry erase pen to help a child gain independence in organizing the "plan" and staying on task. I also added a "I am working towards" area for children that have a behavioural chart or are earning "points" towards a reward. Often this desk-visual Help Box can mimic the one the teacher has on the board, but it allows a child to access it without looking up and around the room- wh
