Adapt Include Move

I created this worksheet to use with my High School Life Skills class when I teach them about emergencies and 911. My goal was to help them have a place for key information (their name, address, key numbers) that they could keep at home. There is an English and Spanish version. For my students that have trouble writing their main information (name and address), I used this website to cut and paste their names and addresses onto the worksheets so that they could just trace the information: https

I used this activity with my HS Life Skills students in Adapted Health while in our SEL unit. This was a fun activity that provided students with a way to identify their emotions when they were done that they could use every day if needed. Students will color in the emotions and then create a meter where they can slide their picture up and down to identify how they are currently feeling. There is some back end work on the adult's part but we really enjoyed how these turned out!
