I distribute this student information sheet on the first day of class. Students fill it out and it provides me with a reference for the rest of the semester including contact information, medical alerts, and interesting facts to get to know my students better! "Get Out of Jail Free" Student incentive passes

I use these passes as incentives for my high school students. Each sheet has six passes that students can use for turning in an assignment late, an extra day on a paper, a trip to the restroom, or a "broken printer," (which equals an extra day on a paper :) ) Students earn these for doing well in class--you can decide based on your students, but ideas include: earning an A on a test, having homework for two weeks in a row, etc. I've uploaded them as a word doc so you can adapt and change it
