Getting to Know your Students Form
I am happy to create a custom form for your needs. You can send this form out and the students/parents fill out together then you upload all your information into a spreadsheet for easy reference throughout the school year. I recommend doing one for at the beginning of each semester. This can be for any grade level, and I can customize in images and videos if you'd like.
