I will customize a form for you which you can send out to student/parent or if you are a school lead/admin you can send to your staff. I can add images, link to videos, or feedback for each question. This is helpful when getting to know your students/parents at the beginning of each semester. I can tailor to exactly what you need.

I am happy to create a custom form for your needs. You can send this form out and the students/parents fill out together then you upload all your information into a spreadsheet for easy reference throughout the school year. I recommend doing one for at the beginning of each semester. This can be for any grade level, and I can customize in images and videos if you'd like.
